How do I complete the Precautionary Safety Measures section of the Confined tab in the Permit to Work (PTW) system, in OHS Online?

Okay! Let's take a closer look at this section. Once the confined space work hazards, for the work being done, have been identified, the necessary checks and controls to any confined space work performed is safe, must be confirmed and checked.

The Precautionary Safety Measures section is where these safety measures are determined and confirmed. The Permit Issuer and Permit Requester work through and confirm the safety measure checks and must make sure that these checks are in place before any confined space work starts.

The Precautionary Safety Measures section is found in the vessel section, after the isolation checks have been determined. This section consists of two parts: 

1. General Precaution Measures - These are standard precaution measures which should be in place for the work being done. These will display automatically, as a default in this PTW tab and any options which are relevant to the work being done must be checked as YES and any options not relevant to the work being done must be checked as NO. 
 2. Specific Precaution Measures - These are particular precaution measures which should be in place, when a specific hazardous confined space work aspect of the work being done is present. These will display (be active) when a particular confined space work hazard, in the nature of contents part of the vessel details section, is selected. If the hazard is not selected, its associated specific precaution measure will not be displayed (remains inactive).

Let's take a look at the general precautionary measures a little more closely:

Now, let's look at the nature of contents part of the vessel details section more closely and see what specific precautionary measures their checks will affect:

1/. Temperature Environment Hazards - this becomes a relevant hazard where confined space entry is required into vessels, tanks or piping which have been at very high temperatures or very low temperatures. This environment needs specific checks in place to reduce the likelihood of injuries from hot surfaces, hot working temperatures or cold surfaces and cold working temperatures. This hazard needs to be checked as YES in the Nature of Contents section. When this is checked in the Nature of Contents section, this set of specific precaution measures will display:

2/. Pressure Environment Hazards - this becomes a relevant hazard where confined space entry is required into vessels, tanks or piping which have been under pressure. This environment needs specific checks in place to reduce the likelihood of injuries from pressurised contents. This hazard needs to be checked as YES in the Nature of Contents section. When this is checked in the Nature of Contents section, this set of specific precaution measures will display:

3/. Acidic Basic Environment Hazards - this becomes a relevant hazard where confined space entry is required into vessels, tanks or piping which have contained acidic or basic contents. This environment needs specific checks in place to reduce the likelihood of serious acidic or basic burns on the person(s) entering the confined space. This hazard needs to be checked as YES in the Nature of Contents section. When this is checked in the Nature of Contents section, this set of specific precaution measures will display:

4/. Low Oxygen Environment Hazards - this becomes a relevant hazard where confined space entry is required into vessels, tanks or piping which have very little oxygen and so present a possible asphyxiating hazard. This environment needs specific checks in place to reduce the likelihood of asphyxiation of the person(s) entering the confined space. This hazard needs to be checked as YES in the Nature of Contents section. When this is checked in the Nature of Contents section, this set of specific precaution measures will display:

5/. Flammable/Explosive Environment Hazards - this becomes a relevant hazard where confined space entry is required into vessels, tanks or piping which have contained contents which are flammable or explosive in nature or whose contents are not known. This environment needs specific checks in place to reduce the likelihood of a fire breaking out or an explosion occurring in the confined space. This hazard needs to be checked as YES in the Nature of Contents section. When this is checked in the Nature of Contents section, this set of specific precaution measures will display:
