Sure thing, let's take a closer look at Stage 4! Once the first three stages have been completed, the PTW needs to be printed to hard-copy for signatures. Stage 4 refers to the completion of this aspect of the PTW:

Signatures Aspect: The contractor confirmation sections of the PTW need to be signed off by both the Permit Issuer (PI) and the Permit Requester (PR) to formalise the identified hazards and associated safety precautions and confirm these are in place. Furthermore, the induction training details need to be signed by each attendee to confirm they attended the training. 

The following tab sections in the PTW hard-copy require signatures: 

PPE tab - this confirms the relevant PPE is in place and needs to be signed by the PR. Environmental tab - this confirms the relevant environmental checks are in place and needs to be signed by both the PR and PI. 
Hot Work tab - this confirms the relevant hot work checks are in place and needs to be signed by both the PR and PI. 
EQMT Isolation tab - this confirms the relevant isolation checks are in place and needs to be signed by both the PR and PI. 
WAH tab - this confirms the relevant height work checks are in place and needs to be signed by both the PR and PI. 
Excavation tab - this confirms the relevant excavation work checks are in place and needs to be signed by both the PR and PI. 
Confined tab - this confirms the relevant confined space entry checks are in place and needs to be signed by both the PR and PI. 
Asbestos tab - this confirms the relevant asbestos work checks are in place and needs to be signed by both the PR and PI. 
Cold Work tab - this confirms the relevant cold work checks are in place and needs to be signed by both the PR and PI. 
Induction tab - this confirms the relevant contractor employees have attended induction training and needs to be signed by each attendee and the trainer(s). 
Approve tab (declaration and approval; section 15) - this confirms all the relevant safety precaution checks are in place and needs to be signed by both the PR and PI.

Print Aspect: In addition to the main PTW being printed to hard-copy, there are a couple of appendix documents which also need to be printed and inserted into the PTW to complete it as a formal document: 

Risk assessment - the complete risk assessment (RA) needs to be printed, using the print function button in the Risk Assessment tab (section 3). This will print the RA in full, allowing it to be added into the PTW. 
Tagout tags - any relevant isolation tagout tags need to be printed, so these can be used at the correct equipment isolation points. Once printed, the relevant sections of the PTW are signed and a copy of the signed PTW given to both the contractor and the client. Once this is done the required work of the contractor can begin.
