As an OHS Online user, what documents do I need to submit for an Injury on Duty (IOD)?
This is another important question! When an IOD occurs, the Compensation Commissioner, requires certain documents to be submitted, which relate to the IOD. These initial documents need to be submitted, in order for the Compensation Commissioner to accept the claim:
- Employers Report of an Accident from the employer (this is also known as a W.Cl.2). In the case of companies under the Rand Mutual Assurance, this would be the RMD.1 report.
- Certified copy of the injured person's ID
- Copy of the injured person's current payslip
- First Medical Report from the doctor (this is also known as a W.Cl.4). In the case of companies under the Rand Mutual Assurance, this would be the equivalent form to the W.Cl.4 (RMA forms are not numbered and differ from WCA forms)
These documents need to be submitted, in order for the Compensation Commissioner to finalise the claim:
- Progress Medical Report from the doctor (this is also known as a W.Cl.5). Where more than one consultation is required with the doctor, this report is known as a Progress Report and new forms need to be submitted on a monthly basis, up until the last consultation.
- Final Medical Report from the doctor (this is also known as a W.Cl.5). This report is known as the Final Medical Report on the employee's last consultation with the doctor.
- Resumption Report from the employer (this is also known as a W.Cl.6) - This report is completed by the employer and relates to certain details about the injured employee.
With the new online system, doctors are to now claim by uploading their accounts directly.
