The W.Cl.5 – Final / Progress Medical Report form

This is a very good question! At first glance, the W.Cl.5 - Final / Progress Medical Report form does appear confusing. However, it's actually two forms in one because it serves two purposes. Firstly, it's a progress report, describing the progress details of the employee as they recover from their injury(ies). Secondly, it's a final report, which checks and confirms that the employee has recovered from their injury(ies) and is fit to return to work. Like the other forms in this process, it is more commonly referred to as a W.Cl.5 form.

The W.Cl.5 form is completed by the doctor and, based on his or her prognosis, will determine whether the employee is fully healed or whether they need to return again for a further consultation. In the case of a further consultation, the report is considered a progress report and the doctor will mark this as such. In the case of the employee being fully healed, the report is considered a final report and the doctor will mark this as such:

 

Now, let's take a look at how the W.Cl.5 differs when it is reported as a progress report and when it is reported as a final report:

 

It is important to make sure that the employee continues to visit the doctor, while the reports return as progress reports. Once the employee returns with the final medical report, the doctor is happy that the employee is fully recovered and he or she will no longer need to visit the doctor.

 

Each time the employee returns from a doctor's visit, make sure that the employee provides you with the associated W.Cl.5 form. Again, make a copy of this yourself and keep this on record and then submit the W.Cl.5 form to the Compensation Commissioner.
